Quarantine certificate or a self-declaration form to be brought along with other documents

Students of the Sri Jayawardenapura University sitting for the second and fourth year examinations have been requested to submit a quarantine certificate or a self-declaration form before they participate in the examinations,  due to the COVID-19 situation.

The students had been requested to obtain a quarantine certificate from the Public Health Officer (PHI) in charge of their area of residence. However if the PHIs refused to grant a quarantine certificate, the students had the option of filling in a self-declaration form and submitting it when they attend the examinations. They had been advised to assemble near the respective examination halls by 8.30 am.

The self-declaration form should indicate if the students had undergone treatment for COVID-19, if any of their family members were infected, if they had associated with any infected family members and details of self-quarantine. Any details of foreign travel after July 15 should also be included. Details of COVID-19 symptoms such as coughing, sneezing, fever or sore throat should also be included.

If the information in the self-declaration forms were false, the students would be liable to be prosecuted and punished under the quarantine rules and regulations of Sri Lanka.
